I'm kind of surprised no one seems to have mentioned MD Geist. Its pretty bad and few have managed to really rival it for badness in my eyes.

Deadman Wonderland??? I haven't watched the anime but the manga itself is pretty good?
Also Serial Experiments Lain tries way too hard to be something special; so much so that it becomes a train wreck of absolute nothingness. A shame, because it looked interesting at first.

It's really hard for me to not just slam Queen's Blade but I'm sure there was something worse i tried to watch. Astarotte's Toy probably.
Also Key, Serial Experiments Lain was something super special when it was made. You were 5 years old when that aired in Japan and not too much older when it originally aired state side on Tech Tv... RIP Tech Tv.

Holy Shit. Diabolik was so awful! Oh and Deadman wonderland manga was awesome, but it might be a touch to gory for anime to translate to well without editing the fuck out of it. And I hate almost all dubbed anime. There are obviously exceptions like Cowboy Bebop, FMA, DB franchise (cause Goku's voice in Japanese is unbearable), and a few others.
