I haven't really tracked down the worst hentai, I'm sure there is some awful lolicon rape incest stuff. Immoral Sisters was pretty bad. A man fakes an accident to blackmail a woman into performing sexual favors and then moves on to her two daughters. Has some pretty disturbing rape scenes.
