I didn't want to put spoilers in the thread, but I have a hypothesis going that SMTIV directly follows SMTII; this storyline could be following the Lawful ending, where the Millenium Kingdom is erected over Tokyo, forsaking everyone beneath. This then results in the world that the people above the surface know, while everyone below was left to their own devices.
